You can make a payment to join or renew your smile club membership($249), as well as add new family members to your plan($219,) using our secure payment form below.
Please provide the name and birthday of the patient, so we can apply the payment to your account. If you are adding a family member, please also provide their name and birthday.
After making a payment, you will be provided with your payment confirmation number and a receipt via email. You will receive your Smile Club Card at your first visit to our office.
